Please complete the following Baptism Application Form if you wish to baptize your child 6 years of age or younger at St. David of Wales Church. We offer the Sacrament of Baptism at any of our regularly scheduled Masses or at our monthly group baptism (outside of Mass) on the second Saturday of every month at 9 a.m. We do not offer private baptisms. In addition to completing this form we need a copy of the child’s birth certificate. Once we receive this application we will contact you to confirm or make you aware of any other needs.
